The Miami-Dade Health Action Network (MD-HAN) is a multi-sector cross collaborative, which includes over 2,300 interested Community Partners, guided by a Steering Committee, serving as the neutral convening platform to address issues regarding the promotion of an integrated safety-net system and access to health care in Miami-Dade County.
In 2008, the Florida Department of Health in Miami-Dade County (FDOH-MDC) in partnership with the Health Council of South Florida (HCSF) and then Florida State Representative Juan Carlos Zapata convened a group of community stakeholders to establish the MD-HAN and address issues regarding access to healthcare.
In 2011, the HCSF understood the importance of continuing MD-HAN efforts to prepare for the upcoming changes in healthcare despite FDOH-MDC budget cuts provided the in-kind project administration for MD-HAN.
In 2012, the HCSF positioned as MD-HAN’s Project Administrator obtained seed funding to enhance the work of the Network and launched the Health Access Initiative (HAI). This initiative engages community stakeholders to focus on reducing uncompensated care and the mis-utilization of emergency departments and healthcare services.
Organizational Structure
The MD-HAN is a four tiered structure, composed of the Transformational Leadership Council, a Steering Committee, four Work Groups and over 2,100 Community Partners countywide.

Health Access Initiative
In 2012, realizing the importance and potential of the Network, HCSF launched the MD-HAN’s Health Access Initiative (HAI). The HAI’s main focus areas during the next two years are to effect positive systemic changes that increase collaboration and enhance the capacity of Miami-Dade’s safety-net to address the needs of the underserved and uninsured.
