Goal: The goal of this program is to help children recognize and deal with anxiety.

Impact: Studies have found that participants in the Coping Cat program show significant reductions in anxiety and fear, improvements in ability to cope with dreaded situations, and a reduction in the frequency of negative thoughts during the week.

Goal: The goals of the MCM are to provide a comprehensive and centralized concussion care program to 1) increase concussion awareness and identification through education and training; 2) facilitate the return to play decision with effective medical treatment, which includes baseline neurocognitive testing; and 3) implement a standardized concussion care protocol and concussion injury surveillance system to assist in the prevention of concussions, improve player safety, and limit school liability.

Impact: From the pilot evaluation of the model it was determined that the MCM model or a similarly designed one is effective in increasing the number of concussions identified, reported, and also treated at a clinic.

Goal: The objective of this article is to describe Project Restoration's approach to cross-sector collaboration with a shared population of high utilizing community members, describe the process and outcomes of the collaboration, provide strategies to mitigate challenges that arise during collaboration and offer insights to inform similar initiatives in communities nationwide.

Impact: Hospitals across the region are assessing ways to establish a centralized infrastructure to collaboratively address the complexities of individual cases, as well as a regional council to explore process improvements to enhance the lives of vulnerable populations.

Goal: The goal of ELSB is to help moderately to severely disabled children develop the skills and behaviors they need to succeed in a standard reading program.

Impact: ELSB demonstrates that reading skills curriculum adapted to alternative instructional needs of cognitively disabled children can more effectively improve literacy as compared to sight-word-only programs.

Goal: The goal of Emergency Department Means Restriction Education is to help parents and adult caregivers of at-risk youth recognize the importance of taking immediate action to restrict access to firearms, alcohol, and prescription and over-the-counter drugs in the home in order to lessen the risk of self harm.

Impact: The Means Restriction program shows that ED-based programs and provided practical information can help parents and adult caregivers of at-risk youth recognize the importance of taking immediate, new action to restrict access to dangers in the home.

Goal: Making Connections for Mental Health and Wellbeing Among Men and Boys is a national initiative to transform community conditions that influence mental wellbeing. The Prevention Institute works with 13 communities across the U.S. to shift policies, practices, and norms to create greater opportunities for health and resilience, with particular focus on veterans and men and boys of color.

Goal: MANNA uses nutrition to improve health for people with serious illnesses who need nourishment to heal. By providing medically tailored meals and nutrition education, we empower people to improve their health and quality of life.

Impact: MANNA members report significant health care cost reductions due to improved health.

Goal: The goal of the Family Peer Support program is to increase family economic and social self-sufficiency, and to connect parents to needed physical health, behavior health, and educational resources for their child. Family peer support programs generally focus on fostering encouragement of personal responsibility and self-determination, improving family health and wellness, and supporting engagement and communication with providers and systems of care. Research shows that peer support programs promote empowerment and self-esteem, self-management, engagement and social inclusion, as well as improving the social networks of families who receive these services. Research evidence qualifies peer support services as evidence-based through the Agency for Healthcare Research and Quality guidelines.

Goal: The mission of the Institute on Violence and Destructive Behavior (IVDB) is to empower schools and social service agencies to address violence and destructive behavior, at the point of school entry and beyond, in order to ensure safety and to facilitate the academic achievement and healthy social development of children and youth. The primary goal of this program is to divert antisocial kindergartners from an antisocial behavior pattern during their subsequent school careers and to develop in them the competencies needed to build effective teacher- and peer-related, social-behavioral adjustments.
