The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

Goal: The goal of this workshop is to build the participants’ self-confidence in their ability to manage their health and maintain active and fulfilling lives despite their chronic disease condition.

Impact: In 2016 Derby Health Collaborative hosted six free self-management education courses in partnership with the Derby School District, the Derby Senior Center, the Derby Public Library and the Kansas State Extension and Research Center. Workshops will continue in 2017.

Goal: The goal of this program is to relieve symptoms of PTSD, depression, and general anxiety among children exposed to trauma.

Impact: Studies have found significant reductions in PTSD and depression symptoms for treatment children in CBITS when compared with a control group. Additionally, the program demonstrated effectiveness at reducing parent-reported psychosocial dysfunction among participating children.

Goal: The goal of this program is to reduce disability in middle-aged and older people with schizophrenia.

Impact: At the end of treatment, CBSST participants had significantly greater cognitive insight scores, indicating more objectivity in reappraising psychotic symptoms relative to treatment as usual. At 1-year follow-up, participants in CBSST showed greater skill acquisition and significant improvements in social functioning relative to participants receiving treatment as usual.

Goal: The goal of Cognitive Behavioral Therapy for Adolescent Depression is to treat depressive symptoms in adolescents.

Impact: Cognitive Behavioral Therapy for Adolescent Depression showed more rapid treatment response than both systematic behavior family therapy and non-directive support therapy. CBT also showed a greater rate of decline in self-reported depression over time.

Goal: The goal of Color Me Healthy is to promote and encourage physical activity and healthy eating among children ages four and five.

Impact: The program has had a positive impact on children's knowledge of and participation in physical activity. Similarly, it has had a positive impact on children's ability to recognize and their willingness to try fruits and vegetables. It has also increased children's fruit/vegetable snack consumption.

Goal: The goal of this program was to increase colorectal cancer screening recommendations and completion rates at a Veterans Affairs medical center.

Impact: Veterans Affairs (VA) patients in the intervention group received more recommendations for colorectal cancer screening and completed more screening tests compared to those in the control group.

Goal: The goal of the Colorectal Web interactive website is to promote colorectal cancer screening.

Impact: Colorectal Web is more effective than a standard colorectal cancer website at prompting previously unscreened individuals to choose a preferred colorectal cancer screening test and to be screened for colorectal cancer.

Goal: The purpose of the Community Supporting Breastfeeding (CSB) designation program is to create a culture of breastfeeding support in communities, ultimately increasing breastfeeding initiation and duration rates and the health and well-being of families.

The CSB program’s immediate goals are to increase positive perceptions toward breastfeeding, increase the availability of breastfeeding resources, and reduce barriers experienced in the community by women of child-bearing age. The long-term goal of the project is to increase exclusive breastfeeding rates through a sustainable model of cross-sector support for breastfeeding.

The project also effectively addresses the racial and socioeconomic disparities in breastfeeding rates. The Kansas Breastfeeding Coalition’s collective impact approach supports efforts of those serving African-American and lower income women. As a result of this project, these organizations receive the support they need to increase their impact when they become aligned with other, larger organizations working toward similar goals, creating a synergy across groups working on breastfeeding support.

Impact: Based on results from our evaluation, along with improved exclusive breastfeeding rates in the majority of the communities that could be associated with the CSB criteria, we believe the CSB program is impactful.

Goal: The clear goal of CARE is to foster projects that will become self-sustaining and use CARE funding as seed money. The CARE Process is designed to encourage communities to enlist the support of project partners that will enable the project to continue even without EPA resources. Rather than supporting one time projects, CARE will support community partnerships that will endure and provide environmental benefits long into the future.
